MY DentalCARE PLAN
My DentalCare Plan is a discount plan offered by Dr. Laila Rizvi to make it easier for our patients to receive comprehensive, preventative dental care. This is not an insurance. It is a patient loyalty reward program. Some of the features and benefits are as follows:
- Easy Payments
- Get comprehensive, preventative dentistry
- No yearly maximum benefits or annual deductibles to meet
- NO claim forms - You pay the discount amount at the time of the visit.
- 15% off ALL dental services provided at the office including root canals, crowns, implants, Invisalign, and all other services (not eligible towards products eg. electric toothbrushes, brush heads, medicaments etc)
My Plan
$400/year
- TWO dental cleanings per year D1110
- Comprehensive exam including x-rays necessary D0150,D0120
- 15% off all other dental services -Including cosmetic procedures
- 2 emergency exams per year and periapical xrays if necessary D0140, D0220
My Child Plan
$350/year
- TWO dental cleanings per year with FLUORIDE treatment D1120,D1206
- Comprehensive/annual exam including x-rays D0150, D0120
- 2 emergency exams per year and periapical xrays if necessary D0140, D0220
- 15% off all other dental services - including cosmetic procedures
MY PerioPlan
$900/year
- Up to FOUR periodontal maintenance cleanings per year and antibacterial irrigation D4910
- Comprehensive exam including x-rays necessary D0150,D0120
- 2 emergency exams per year and periapical xrays if necessary D0140, D0220
- 15% off all other dental services - including cosmetic procedures
MY FAMILY Plan
$1400/year
- Up to FOUR individual plans Adult prophy or Child prophy
- Perio plans cannot be included in family plans
- 15% off all other dental services - including cosmetic procedures
